Code of Conduct Reporting Form
Cold Spring Harbor Laboratory Meetings & Courses Program

The CSHL Meetings & Courses Program sincerely appreciates all reports of breaches in our Code of Conduct, as they help us strengthen campus safety and the wellbeing of staff members, scientific visitors, and participants in our meetings and courses. The full Code of Conduct is on the CSHL website as well as in printed materials such as welcome folders and abstract books.

You may use this form to submit reports of such breaches electronically, or you can submit them directly to either (or both) of the following:

  • Dr. David Stewart, Grace Auditorium Room 204, 516-367-8801 or x8801 from a campus phone, stewart@cshl.edu

  • Dr. Charla Lambert, Hershey Laboratory Room 214, 516-367-5058 or x5058 from a campus phone, clambert@cshl.edu

Reports can be submitted by those who experience harassment or discrimination, either in person at CSHL or online during a CSHL virtual event, as well as by those who witness violations of the behavior laid out in the Code of Conduct.

All fields below are optional. This means you can use this form to submit a report anonymously. Please note that, in many cases, anonymous reports preclude the possibility of a formal follow-up investigation of the reported incident. If you do enter your name below, CSHL will follow up with you; if you also include the name of the person who violated the Code of Conduct, CSHL may be able to undertake a formal investigation of the incident.

Regardless of how much information you know or choose to include here, your report is extremely valuable.  We will treat it with the utmost concern and confidentiality, and we will ensure that you (or the victim) are in control of and comfortable with any follow-up actions we may take.
